What To Bring To Your Pasco County Tax Collector's Office
Pasco County Driver License Document Checklist
TAX COLLECTOR / PASCO COUNTY / FLORIDA
1. Identification - Bring ONE of the following:
- Certified U.S. Birth Certificate (Issued by government agency; hospital birth certificate not accepted).
- Valid U.S. Passport.
- Certification of Naturalization.
- Consular Proof of Birth Abroad.
- Alien Registration Receipt Card (Green Card, Form I-551).
2. Social Security Number - Bring ONE of the following:
- Social Security Card.
- W-2 Form (preprinted, cannot be handwritten).
- Paycheck showing complete number.
- IRS Form 1099-MISC.
3. Residential Address - Bring TWO of the following:
(Proof is not limited to the list below and cannot be a post office box.)
- Deed, Mortgage, Payment Booklet or Rental Agreement. *Must be dated within the last 60 days.
- Florida Vehicle/Vessel Registration or Title (Cannot accept two documents from same DMV record).

4. Name Change - if applicable:
(Include any documents needed to support any and all name changes since issuance of original birth document.)
- Original or certified copy of all marriage certificates or court orders that show your name change(s).
(Certified document issued by government agency; church certificate not accepted.)
NOTE: An honorably discharged US Veteran may have a Veteran Designation "V" placed on their driver license by providing a copy of their DD214 or other approved discharge document.
