Hunters: Recreational Use Permit Applications will be accepted Friday, June 21 at our Gulf Harbors Office ONLY.
Office will open for application drop-off at 7:30 am.

TO TRANSFER FROM A FLORIDA LEARNER'S LICENSE TO A
FLORIDA DRIVER'S LICENSE
To Transfer a Florida Learner's License to a Florida Driver's License the following is required:
       1.  United States Birth Certificate OR valid United States Passport. A birth certificate must be issued by a government agency. Hospital birth certificates cannot be accepted.
       2.  Social Security Card, or one of the following documents showing your full social security number: W-2 form, pay check stub or Form 1099.
            The name assigned to the social security number must match the name that will appear on the Florida Driver License.
            If you have recently changed your name, update your records with the Social Security Administration
            before you apply for your Florida Driver License.
       3.  You must be at least 16 years old and have held a Learner's License for at least one year without any traffic convictions.
       4.  Provide a Florida Learner's License.
       5.  Parental Consent Form either signed and notarized in advance, or signed in the presence of our examiner, certifying that the
            applicant has had a minimum of 50 hours of behind-the-wheel driving experience, 10 hours of which was night driving. Person signing
            Parental Consent Form must be legal guardian. If other than parent, such as step or grandparent, proof of guardianship must be provided.
       6.  Vision Test.
       7.  Driving test, which must begin no later than one hour before office closing time.
       8.  Proof of residential address requires two of the following reflecting your name and address: deed, mortgage statement, lease agreement,
            voter registration, vehicle registration, W2 form, 1099 form, or a homeowners, automobile, or life insurance policy.

       9.  If the documentation submitted reflects any name change, an original or certified copy of your Marriage License, Divorce Decree or Court
            Order, must be submitted. In some instances, more than one marriage license, divorce decree or court order document may be required
            to show the chain of name changes. Photocopies cannot be accepted. Marriage license must be from a governmental agency. Church issued
           licenses cannot be accepted.
