NOTE: Effective September 1, 2009, most motor vehicle and driver license fees increased significantly. These fee increases were passed by the Florida Legislature and approved by the Governor to help balance the state budget. None of the additional money will stay with Pasco County Government or the Tax Collector's Office. FAQS for First Time Driver License and Identification Card Changes Effective January 1, 2010
To Transfer a Valid Out of State License to Florida:
    The following is required to transfer a valid out of state license to a Florida Driver License:
       1.  United States Birth Certificate OR valid United States Passport. A birth certificate must be issued by a government agency. Hospital birth certificates cannot be accepted.
       2.  Surrender of the out of state license.
       3.  Social Security Card, or one of the following documents showing your full social security number: W-2 form, pay check stub or Form 1099.
            The name assigned to the social security number must match the name that will appear on the Florida Driver License or
            Identification Card. If you have recently changed your name, update your records with the Social Security Administration
            before you apply for your license or Identification Card.
       4.  Vision Test.
       5.  Proof of residential address requires two of the following reflecting your name and address: deed, mortgage statement,lease agreement,
            voter registration, vehicle registration, W2 form, 1099 form, or a homeowners, automobile, or life insurance policy.

       6.  If the documentation submitted reflects any name change, an original or certified copy of your Marriage License, Divorce Decree or Court
            Order,must be submitted. In some instances, more than one marriage license, divorce decree or court order document may be required
            to show the chain of name changes. Photocopies cannot be accepted. Marriage license must be from a governmental agency. Church
            issued licenses cannot be accepted.
    If the out-of-state license is expired, suspended or revoked, a written and/or road test may be required.

To Apply For a First-Time Florida Identification Card:
    The following is required to apply for a Florida Identification Card:
       1.  United States Birth Certificate OR valid United States Passport. A birth certificate must be issued by a government agency. Hospital birth certificates cannot be accepted.
       2.  Social Security Card, or one of the following documents showing your full social security number: W-2 form, pay check stub or Form 1099.
            The name assigned to the social security number must match the name that will appear on the
            Identification Card. If you have recently changed your name, update your records with the Social Security Administration
            before you apply for your Identification Card.
       3.  Applicant must be 5 years of age or older. If the purpose of the Florida Identification Card is to apply for a Disabled Parking Permit,
            the age limit is waived. A certification of the disability must be provided.
       4.  Proof of residential address requires two of the following reflecting your name and address: deed, mortgage statement, lease agreement,
            voter registration, vehicle registration, W2 form, 1099 form, or a homeowners, automobile, or life insurance policy.

To Apply for a First-Time Florida Driver License (also known as a Learner Permit) and
the applicant has never been licensed by any state or county:
    The following is required to apply for a Learner Permit:
    For applicants 17 years or younger:
       1.  United States Birth Certificate OR valid United States Passport. A birth certificate must be issued by a government agency. Hospital birth certificates cannot be accepted.
       2.  Social Security Card or one of the following documents showing your full social security number: W-2 form, pay check stub, or Form 1099.
            The name assigned to the social security number must match the name that will appear on the Florida Driver License.
            If you have recently changed your name, update your records with the Social Security Administration
            before you apply for your license.
       3.  Proof of completion of a traffic law substance abuse education course.
       4.  Parental Consent Form either signed and notarized in advance, or signed in the presence of our examiner. Person signing the
            consent form must be legal guardian. If other than parent, such as grandparent, proof of guardianship must be provided.
       5.  Vision Test.
       6.  20-Question Road Rules Test and 20-Question Road Sign Test. Tests must begin no later than one hour before office closing time.
       7.  If using an On-Line Third Party Tester, a Parent/Guardian On-Line Test Proctoring Form must be signed in the presence of a driver
            license examiner by the person who proctored the test; or the proctor's signature must be notarized if the parent or guardian proctor
           will not be present.
       8.  Proof of residential address requires two of the following reflecting your name and address: deed, mortgage statement, lease agreement,
            voter registration, vehicle registration, W2 form, 1099 form, or a homeowners, automobile, or life insurance policy.
